Registration Fee: $20 Early Bird before September 30 / $25 General before October 25 / $15 Crohn's & Colitis Foundation Professional Members or Chapter Medical Advisory Committee Members. Group Rate! $15 each when you sign up 4 or more attendees from the same office. Please register separately. Refreshments and lunch included. Street parking is free on weekends.
Registration deadline: Wednesday, October 25, 2017. For those who miss the pre-registration deadline, you may sign up at the event for a $30 registration fee. Registration opens at 8:30 AM.
Accreditation: This activity is expected to be accredited for approximately 4.75 continuing education hours.
The lot under the CHH will be closed for construction. Please park in Lot 33 along SW Moody Ave between SW Curry Ave and SW Gaines St-- see the map. Our group has been given permission to park here for free for the day. There is paid street parking but it is time-limited and is not a good option for the full day.
From the main lobby, take elevators up to the 3rd floor. Go right down the hallway and the class will be at the end of the hallway.
